MCQs Submission

This form is to set up the quiz which users need to answer to be awarded the certificate and subsequently be able to apply for the CME points. The quiz consists of multiple choice questions with 4 possible answers. Each question can have only one correct answer. If true/false or correct/incorrect choices are chosen, the other 2 possible answers can be both/none etc. At the end of each question, there is a space for the learning point that is feedback behind each answer and information on the correct answer.
